Please print the Patient Forms below, fill them out and bring them with you to your first scheduled appointment.  This will include a health history form, our privacy notice and a form acknowledging you read the privacy form. 

If you have any current radiographs from a previous dentist, a copy of these is very helpful during an initial exam.  Due to privacy laws, you must request these records yourself.  We are unable to request them for you.
