Please print the New Patient Form and Privacy Acknowledgement below and bring with you to your first scheduled appointment.  Our Privacy Policy is located below and may be read online or printed for your perusal.  Also, if you have any current radiographs from a previous dentist, a copy of these is very helpful during an initial exam.  Due to privacy laws, you must request these records yourself.  We are unable to request them for you.
